What Thrive includes

• An initial 60–90 minute consultation to understand your symptoms, history and goals.

• An individualized care plan and education to help you understand your options.

​

• Up to six follow-up visits per year, lasting 15–30 minutes each.

​

• Symptom tracking and review of your plan as your needs change.

Follow-up frequency is based on your needs; not everyone will require all six visits.

Your membership

$300 one-time sign-up fee
Includes your initial 60–90 minute consultation.

$75 per month
A minimum six-month commitment applies.
